Use this form to document a concern and connect it to the adult-supported 5041 ROAR process. If you are unsure whether something qualifies, it is okay to report it.

Before submitting I understand that 5041 will keep this information private to the greatest extent reasonably possible, but an adult may need to share it with school officials, parents/guardians, Title IX personnel, law enforcement, emergency services, or others when required for safety, law, district policy, or a fair review.

An appropriate adult listens without judgment and documents what is being reported rather than deciding the outcome immediately.

Qualified adults review severity, frequency, context, impact, safety, available evidence, and whether school procedures must control.

Respond

The appropriate support, protective action, parent/guardian involvement, school referral, or other response is used.

Student leader boundary: Student ROAR Leaders may listen and connect a student with an adult. They do not investigate, interview witnesses independently, decide guilt or discipline, or promise confidentiality.

Talk to a ROAR Mentor Representative

You can report directly to an adult ROAR representative rather than using the website.

Talk to the faculty advisor or a school adult

Especially use a school reporting channel when the concern involves harassment, discrimination, sexual misconduct, threats, or safety.

Talk to a Student ROAR Leader

A student leader can listen and help connect you to an adult, but the student leader will not investigate the concern.
